Actually, no, I have not. When in a fight, I only click on blank space (because the link to the weapon of choice moved due to formatting, or the link to attack using the weapon I want.
I did click somewhere else once when in my early life. I didn't make that mistake again..
When attacking, don't alt-tab, switch browsers or tabs, or click anything else but your next attack.
You can do all that stuff later when you are not in a fight.
