Also, I want to be able to give people who're in the hospital staff infections with unsanitary environments, or cut them open a little more during surgery, so they have a lengthened hospital stay, please.

Yes, and like wrecking balls for construction (takes a ton of cps, but takes out half the hideout of the opposing gang), and "barriers" (or think of something better) for engineering, that prevents damage, again costing a lot, but preventing hundreds of damage at once.
Both of these ideas were discussed I think about a year ago. OK, if they're not implemented as SV wanted, but I think every gang would like a new ability for their members' careers eventually, and still....nothing!

If/When turrets are implemented, they could improve damage/defense of the turrets.
